Important notes about completing the form

You must list all accounts, credit cards, and financial assets or liabilities—even if the balance is $0. Everything must be disclosed. This includes closed or inactive accounts, as they may still appear on financial records and need to be accounted for.

If you don't have exact figures: An informed estimate is perfectly acceptable—just note that it is an estimate, and we can adjust it later if needed.

Income

In addition to the SNW, please gather and provide copies of the following:

  • Your most recent W-2s, 1099s, and K-1s
  • Federal and State Income Tax Returns

Expenses

Unless otherwise stated, please list your expenses on a monthly basis. If you only know the annual total for something (for example, insurance premiums), divide by 12 and note that it is an estimate.

  • For monthly expenses like groceries, vacations, clothing, vehicle repairs, and unreimbursed medical, dental, and optical costs, calculate a 12-month average.
  • For gas and electric bills, please contact your utility provider and request a 12-month billing summary.
  • If you pay for water, request a 12-month billing summary from the water company as well.
  • If your cable, internet, phone, and cell phone bills are consistent each month, you can use the exact monthly amount. If they fluctuate, please use an average.
  • For expenses such as gas for your car or meals out, if you spend about the same amount each week, multiply the weekly amount by 4.333 to account for months with more than four weeks.
  • Any other expenses (e.g., Amazon Prime, Netflix) can be listed in the "Other" section at the end.

Assets and Liabilities

  • For the assets and liabilities sections, please include current statements. The "balance as of date of commencement" refers to the date the divorce action began; in your case, we will use today's date.
  • Please be sure to list all outstanding debts and liabilities, including credit card balances, personal loans, lines of credit, and any loans you have co-signed.
  • If you own real estate, please include the address, type of property (e.g., primary residence, rental), estimated market value, outstanding mortgage balance, and monthly mortgage payment.
  • If you have any life insurance policies, indicate the type (term or whole life), the face value, and the named beneficiaries.
  • If any assets are your separate property (for example, a retirement account you held before marriage), please note this clearly on the SNW. If you are unsure whether something qualifies as separate property, feel free to contact us so we can help you identify any potential claims.
  • For jointly owned assets or debts, please indicate who holds title (you, your spouse, or both).
